Using this form is not required. You may also declare your withdrawal by any other clear statement, for example by email or letter. This form is a convenience, not a prerequisite.
Withdrawal is not the same as cancellation: withdrawal is the exercise of the statutory right of withdrawal, while cancellation ends ongoing coaching under the agreed terms.
